Medical Records

Access to medical records is available to patients over the age of 18 or legal guardian, and is protected by Federal HIPAA regulations. Copies of records may be released upon receipt of written authorization of the patient or guardian.  Release forms are available at the hospital front desk.
 

There is a charge of $1.00 per sheet for the first 25 sheets, and .50 per sheet thereafter. 
 

Once authorization is received, it may take up to 10 days to process your request.

Records for Physician

If your physician is on staff at Bibb Medical Center, he or she may access your medical records from their office without your permission. If not, your doctor’s office can be faxed records that you personally request at no charge to you. You must submit the appropriate authorization form and include the physician’s name, mailing address, phone number and fax number.

Medicaid Patient Records

Medical records needed for a Medicaid Patient must have a signed approval from Medicaid on file before any records may be release. Medicaid release forms may be obtained from the hospital front desk, the Alabama Medicaid website or you may call Medicaid at 800-688-7989.

Insurance, Attorney and Disability Request

Request should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address on the authorization form. All charges for medical records will be billed to the requester.

Radiology/Imaging Films

Please contact the Radiology Department directly at 205-926-3257 to receive copies of these records.

Death Certificates

Death certificates must be requested from the Health Department at 205-926-9702.
