BMC at Home wants to increase access to your healthcare provider by having our medical team come to you. The BMC at Home team will provide comprehensive medical, social and behavioral care.
This program is designed to provide compassionate and skilled primary healthcare while remaining in the comfort of your home. Our team will assist with all of your healthcare needs, such as annual wellness visits, routine and medication refill appointments, sick visits and preventative services. BMC at Home will also provide educational opportunities to promote self management of chronic health conditions.
As a BMC at Home patient, you will receive a personalized care plan to help manage your chronic conditions at home while optimizing your overall health.
